Description
Arising segmentally from the posterior aspect of the abdominal aorta, the lumbar arteries course laterally and posteriorly across the anterolateral surfaces of the lumbar vertebral bodies before passing deep to psoas major toward the posterior abdominal wall. From a posterior perspective, their relationship to the lumbar spinal nerves is clear, with the vessels crossing near the intervertebral foramina as the ventral rami emerge and continue laterally. Kidneys sit superolateral to the upper lumbar levels, while the iliac crest and superior pelvis frame the inferior field. Small branches track toward quadratus lumborum and the paraspinal musculature. Short communicating veins accompany the arteries. This view matters when you need to orient retroperitoneal vascular anatomy to the bony spine and the exiting nerve roots. Lumbar arteries are a common source of ongoing hemorrhage in lumbar spine trauma and can require selective catheter angiography and embolization, where recognizing their segmental origin and posterior course helps avoid non target embolization to radiculomedullary contributors. Surgeons also encounter these vessels during lateral and posterior approaches to the lumbar spine, and inadvertent injury can lead to deep retroperitoneal hematoma that presents late.
