Description
Arising from the posterior interosseous artery, the recurrent interosseous artery ascends proximally along the posterior aspect of the proximal forearm toward the elbow joint, coursing deep to the extensor muscle mass and close to the posterior surface of the ulna. The vessel tracks superiorly toward the lateral epicondyle region, where it participates in the periarticular arterial network, anastomosing with branches from the profunda brachii and radial collateral systems. Surrounding landmarks in a posterior perspective logically include the triceps brachii tendon inserting on the olecranon, the anconeus laterally, and the extensor carpi ulnaris and extensor digitorum more distally, with the humerus, radius, and ulna forming the osseous frame. Red arterial channels and blue venous companions continue distally into the hand over the metacarpals and phalanges. Small, dense anatomy. Posterior elbow vascular anatomy matters when exposure or trauma threatens the collateral circulation that maintains perfusion around a flexed joint. The recurrent interosseous artery becomes clinically relevant in posterior approaches to the elbow and proximal ulna, including open reduction and internal fixation of olecranon and proximal ulnar fractures, where dissection through the extensor compartment can place periarticular vessels at risk. Its contribution to the elbow anastomosis also helps explain why distal limb perfusion can remain acceptable despite proximal arterial interruption, yet still fail when multiple collateral channels are compromised.
