Description
Cut cranial bone margins frame an internal view of the calvaria in an adult male, with the dura mater lining the endocranial surface as a pale, firm membrane. The periosteal (endosteal) layer lies directly deep to the inner table of the frontal, parietal, and temporal bones, while the meningeal layer continues medially toward the falx cerebri and inferiorly toward the tentorium cerebelli. Superficial cerebral gyri and sulci sit deep to the dura, separated in life by the arachnoid mater and subarachnoid space. Tight adherence to bone is the point. That bone to dura relationship matters in trauma and in the operating room. In epidural hematoma, blood dissects between skull and periosteal dura, classically from a middle meningeal artery laceration at the pterion, creating a lens-shaped collection that can rapidly elevate intracranial pressure. Surgeons also exploit the same plane during craniotomy, elevating dura off the inner table while protecting bridging veins and avoiding inadvertent entry into a dural venous sinus along the superior sagittal sinus groove.
