Description

Presented from a posterior approach, the paired kidneys sit retroperitoneally on either side of the vertebral column, with the renal hila oriented anteromedially toward the great vessels. The abdominal aorta descends slightly left of midline, giving rise to the right and left renal arteries that course laterally to the renal hilum, while the inferior vena cava lies to the right and receives the renal veins, which traverse medially from each hilum. Fine segmental branching toward the renal parenchyma is suggested near the hilum, with adjacent yellow neural elements consistent with the renal plexus and green channels consistent with regional lymphatics. Posterior renal vascular anatomy matters because it is where exposure, control, and complication avoidance live in the operating room. During open or laparoscopic nephrectomy, hilar dissection requires clear distinction between renal artery and renal vein, and this view reinforces the classic asymmetry: the right renal artery typically passes posterior to the inferior vena cava on its way to the right kidney. Short and unforgiving. The same spatial relationships underpin renovascular hypertension from atherosclerotic renal artery stenosis, and they help explain patterns of hemorrhage and hematoma tracking in retroperitoneal trauma.
