Description

Posterior dissection of the upper abdomen centers on the paired adrenal (suprarenal) glands perched on the superomedial poles of the kidneys, with the inferior suprarenal arteries ascending from the renal arteries to enter the inferior aspect of each gland. The abdominal aorta lies anterior to the vertebral column and gives rise to the renal arteries coursing laterally toward the renal hila, while the inferior vena cava runs to the subject’s right, receiving the renal veins. Superiorly, the diaphragmatic crura and lower lung bases frame the field, and the liver occupies the right upper quadrant, partially overlying the right kidney and suprarenal region. Attention to the inferior suprarenal arteries matters because adrenal perfusion depends on multiple small feeders that can be overlooked in retroperitoneal surgery and interventional work. During adrenalectomy, nephrectomy, or renal artery reconstruction, these branches often tether the gland to the renal pedicle and can bleed briskly if divided without control. This is also the vascular pathway most likely to be altered when renal artery stenosis or accessory renal arteries change flow patterns, a point that helps explain variable enhancement on CT angiography and the technical challenge of selective adrenal arterial embolization. For teaching, this posterior perspective fits retroperitoneal anatomy sessions in medical and PA gross lab, urology and endocrine surgery modules, and radiology correlation for axial CT at the level of the renal hila and adrenal glands.
