Description
Posterior abdominal exposure centers on the inferior suprarenal artery as it arises most commonly from the renal artery and ascends to the inferior pole of the suprarenal (adrenal) gland, breaking into fine arteriolar branches over the gland’s capsule. Medial to this field runs the abdominal aorta with nearby posterior branches, while the inferior vena cava and renal vein occupy a more right-sided, anteriorly placed venous plane relative to the posterior wall. Autonomic nerves from the celiac and aorticorenal plexuses course alongside the vessels, with small lumbar lymph nodes clustered paraaortically and paracavally. Laterally, the renal hilum and perirenal tissues frame the vascular pedicle. Posterior perspective matters here because the inferior suprarenal artery is the most surgically encountered adrenal feeder during mobilization of the kidney and adrenal, and it varies widely in number, caliber, and origin. Ligation or inadvertent traction on these short branches can contribute to troublesome bleeding during laparoscopic or open adrenalectomy, and their proximity to the renal artery helps explain why partial nephrectomy and renal hilar dissection may place adrenal perfusion at risk. Small, easy to miss vessels. The accompanying sympathetic fibers also clarify why adrenal and renal procedures can provoke autonomic responses and why meticulous tissue handling around the aorticorenal ganglia reduces postoperative neuralgia.
