Originating at the inferior border of teres major, the axillary vein ascends through the axilla medial and anterior to the axillary artery, then becomes the subclavian vein at the lateral border of the first rib. Proximally, it receives the paired brachial veins and basilic vein from the arm, with tributaries consistent with the circumflex humeral and subscapular venous pathways tracking along the scapula and proximal humerus. Superior to the vessel, the clavicle bridges the sternum and scapula, while the rib cage forms the thoracic inlet deep to the shoulder girdle. Nerve elements of the brachial plexus can be appreciated in the same corridor, typically arranged around the artery and closely related to the vein in the medial axillary region. A superior angle on the axillary vein clarifies why it is a common target and a common hazard. For central venous access using an infraclavicular axillary approach, appreciating the vein’s larger caliber, its transition into the subclavian vein, and its proximity to the first rib and pleural cupola helps frame risks such as pneumothorax and arterial puncture. Lymphadenopathy, postoperative scarring after axillary dissection, or effort thrombosis (Paget-Schroetter syndrome) can narrow this venous channel and produce upper extremity swelling, so the topographic relationship to the clavicle, pectoral region, and scapular musculature matters.
