Description
Running superficially along the lateral border of the male foot, the lateral marginal vein begins from the lateral end of the dorsal venous arch and tracks inferior to the lateral malleolus before continuing proximally as the small saphenous vein on the posterior aspect of the leg. Adjacent on the dorsolateral foot, you would expect to see tributaries draining the lateral dorsal digital veins and dorsal metatarsal veins, with the vein lying superficial to the extensor tendons and dorsal fascia. Posterior to the lateral malleolus, the sural nerve typically accompanies the small saphenous vein, while the calcaneal (heel) region sits inferior and posterior as a consistent surface landmark. Orientation matters. Clinically, this lateral superficial venous pathway is a frequent site for venous cannulation when dorsal foot access is required, and it becomes conspicuous in dependent edema, chronic venous insufficiency, and post-thrombotic change. Its relationship to the sural nerve explains a familiar complication: sensory disturbance along the posterolateral leg and lateral foot after small saphenous vein procedures, lateral hindfoot incisions, or sclerotherapy. The course behind the lateral malleolus also provides a dependable reference during lateral ankle approaches when preserving superficial veins and cutaneous nerves reduces wound problems.
