Lateral Perspective of the Lesser Saphenous Vein

Description

Running in the superficial fascia along the posterolateral calf, the lesser saphenous vein (small saphenous vein) ascends from the lateral margin of the foot, passes posterior to the lateral malleolus, and tracks superiorly between the heads of gastrocnemius over the soleus. A lateral perspective also brings the fibula into clear relief, with the vein remaining superficial to the deep posterior compartment and separated from the tibia, which lies more medial. Proximally, the vessel approaches the popliteal fossa to terminate most often in the popliteal vein at the saphenopopliteal junction. Surface anatomy matters here. Venous reflux in the small saphenous vein is a common driver of posterior calf varicosities, and this lateral course helps explain why symptoms and visible dilatations cluster behind the lateral malleolus and along the midline of the calf. Endovenous thermal ablation or ultrasound guided foam sclerotherapy typically targets the trunk while protecting the sural nerve, which runs close to the vein in the distal to mid calf and is the reason numbness along the posterolateral leg and lateral foot is a known complication. Anatomy variants are also clinically relevant, including cranial extension into the Giacomini vein with drainage toward the great saphenous system, which can change procedural planning and duplex reporting.
