Description
Running superficially along the posterior midline of the calf, the lesser saphenous vein (small saphenous vein) ascends from the lateral margin of the dorsum of the foot, passes posterior to the lateral malleolus, and tracks in the subcutaneous tissue toward the popliteal fossa. Proximally it typically accompanies the sural nerve, then pierces the deep fascia between the heads of gastrocnemius to terminate in the popliteal vein. Deep to this superficial channel, the illustration logically places the gastrocnemius and soleus over the posterior tibial and fibular (peroneal) vessels, with the hamstring tendons framing the superior border of the popliteal region. Short perforating veins can be expected to connect the superficial system to the deep veins along the calf. Posterior visualization of the small saphenous vein matters because its termination is variable, and that variability drives both symptoms and procedure planning. Reflux in the saphenopopliteal junction is a common source of posterior calf varicosities, and endovenous ablation or surgical ligation depends on clear appreciation of where the vein dives deep relative to the gastrocnemius heads and popliteal neurovascular bundle.
