Description

Running superficially along the posterolateral margin of the foot, the lesser saphenous vein (small saphenous vein) ascends posterior to the lateral malleolus and continues in the midline of the posterior leg over the calcaneal tendon toward the calf. Proximally it tracks in the superficial fascia between the heads of gastrocnemius, with the soleus forming the deeper muscular bed and the Achilles tendon lying inferior and central as a strong longitudinal landmark. Tributary venules and superficial venous branches converge from the lateral and medial aspects of the leg, while the great saphenous vein remains more medial in position and distinct from the small saphenous course. Posterior mapping of the small saphenous vein matters because this vessel frequently varies in termination, most often draining into the popliteal vein at the saphenopopliteal junction, and it is a common source of posterolateral calf varicosities when the terminal or perforator valves fail. Venous reflux in this territory can be missed if you focus only on the medial leg, so correlating the vein’s surface path with the bulk of gastrocnemius helps when planning duplex ultrasound marking, ambulatory phlebectomy, or endovenous ablation. Orientation is everything.
