Description
Running superficially in the posterior leg, the small saphenous vein (lesser saphenous vein) begins lateral to the calcaneal tendon from the lateral marginal vein of the foot and ascends in the midline of the calf over the gastrocnemius and soleus toward the popliteal fossa. Proximally it lies posterior to the knee joint capsule and between the medial and lateral heads of gastrocnemius, with the hamstring tendons visible superiorly, biceps femoris laterally and semitendinosus and semimembranosus medially. Thin arterial companions and smaller venules cross the field, while white tendinous bands mark the distal hamstring insertions and the fascial planes that keep superficial and deep venous systems distinct. Posterior views matter because this is where you follow the small saphenous vein to its most variable landmark, the saphenopopliteal junction, often located a few centimeters proximal to the popliteal crease and prone to anatomic variation and duplicated channels. Reflux in the small saphenous system commonly feeds posterior calf varicosities and can be missed if attention stays on the great saphenous territory. Nerve proximity is the trap. The sural nerve often accompanies the vein in the distal and mid-calf, a relationship that drives preoperative mapping and informs complication counseling for endovenous ablation or phlebectomy.
