Description
Originating from the lateral marginal vein of the foot, the small (lesser) saphenous vein ascends posterior to the lateral malleolus and tracks superiorly in the superficial fascia along the midline of the calf, closely related to the sural nerve. A posterior view of both lower legs typically also places the gastrocnemius bellies as the dominant muscular backdrop, with the vein running between them before curving into the popliteal fossa. Proximally, its termination into the popliteal vein at the saphenopopliteal junction sits posterior to the knee joint, with venous valves implied along the course and commonly clustered near the junction. Short perforating veins may be suggested as oblique connections from the superficial system to the posterior tibial and fibular (peroneal) deep veins. This perspective matters because small saphenous reflux is a common source of posterior calf varicosities, and mapping the vein’s relationship to the popliteal fossa guides endovenous ablation and surgical ligation planning. The sural nerve runs in the same neighborhood, so a clear depiction of their proximity is more than academic. Variant drainage into the great saphenous system via the Giacomini vein or a high, duplicated junction can change procedural strategy and explains some recurrences after treatment.
