Lesser Saphenous Vein in the Body

Description

Running along the posterior calf, the lesser saphenous vein begins in the lateral marginal vein of the foot, then ascends behind the lateral malleolus and tracks proximally in the superficial fascia toward the popliteal fossa. Midline in the popliteal region, it typically pierces the deep fascia between the heads of gastrocnemius to drain into the popliteal vein, while the great saphenous vein remains more anteromedial along the tibia toward the femoral vein. Perforating veins and tributaries from the posterior leg converge on it, and venous valves are positioned to direct flow proximally against gravity. For teaching lower-limb venous anatomy, this is the cleanest pathway to explain why reflux patterns differ between the small and great saphenous systems. In chronic venous insufficiency, incompetence at the saphenopopliteal junction or along the lesser saphenous trunk produces posterolateral calf varicosities, and ultrasound mapping must account for frequent anatomic variants such as high termination into the femoral vein via the Giacomini vein. Nerve proximity matters too. The sural nerve often travels adjacent to the lesser saphenous vein in the distal calf, a practical consideration during endovenous ablation or surgical ligation.
