Posterior Perspective of the Lesser Saphenous Vein

Running within the superficial fascia of the posterior calf, the small (lesser) saphenous vein ascends from the lateral margin of the foot and posterior to the lateral malleolus, then tracks superiorly in the midline toward the popliteal fossa. Its course lies posterior to the tibia and fibula and inferior to the femoral condyles, with the knee region framed by the patella anteriorly and the distal femur articulating with the proximal tibia and fibula. Venous valves are suggested along the vessel’s length, oriented to direct blood proximally against gravity. A short saphenopopliteal segment is implied near the posterior knee where the small saphenous typically drains into the popliteal vein. Posterior mapping of the small saphenous vein matters because this is the territory for symptomatic varicosities and reflux that are often missed when attention stays on the medial great saphenous system. Endovenous ablation, ultrasound-guided foam sclerotherapy, and phlebectomy planning all depend on correctly identifying the vein’s relationship to the popliteal fossa and its frequent anatomical variation at the saphenopopliteal junction.
