Description
Running subcutaneously along the posterior aspect of the leg, the lesser saphenous vein (small saphenous vein) arises from the lateral marginal vein of the foot just posterior and inferior to the lateral malleolus. It courses superiorly in the midline of the calf over the gastrocnemius, accompanied proximally by the sural nerve, and approaches the popliteal fossa to drain most often into the popliteal vein at the saphenopopliteal junction. Deep skeletal landmarks provide orientation: the pelvis superiorly, femur and patella at the knee, and tibia and fibula framing the crural segment down to the ankle. Superficial venous tributaries are shown converging toward the main trunk. Clear relationships. A posterior leg view matters because the small saphenous vein is a common source of symptomatic varicosities and reflux that are easily missed when attention stays on the great saphenous territory medially. Accurate depiction of the saphenopopliteal junction helps in pre-procedural mapping for endovenous ablation or surgical ligation and in anticipating anatomic variants such as a high termination or continuation as a vein of Giacomini into the posterior thigh. The proximity to the sural nerve also explains a specific complication, sensory disturbance along the posterolateral calf and lateral foot after thermal ablation or phlebectomy.
