Description
Originating from the lateral marginal vein of the foot, the lesser saphenous vein (small saphenous vein) tracks proximally along the posterolateral aspect of the ankle and leg, then swings toward the midline as it approaches the popliteal fossa. A medial perspective can still capture its deeper course relative to the calcaneus, talus, and distal tibia and fibula, with the vessel seen superficial to the crural fascia before it pierces the deep fascia near the knee. Adjacent arterial pathways in the foot and ankle, along with the tarsals, metatarsals, and phalanges, provide firm skeletal landmarks for orienting the venous return. Clean relationships. Clinical teaching often focuses on the great saphenous vein, but the lesser saphenous vein matters when symptoms localize to the posterior calf or when reflux originates from the saphenopopliteal junction. Varicosities in this territory, perforator incompetence, and post-thrombotic changes can complicate duplex mapping because the termination is variable and may connect to the popliteal vein, gastrocnemial veins, or a thigh extension (vein of Giacomini). Endovenous ablation and surgical ligation both depend on recognizing where the vein leaves the superficial plane and how it relates to structures at the popliteal fossa, including the tibial nerve and the popliteal vessels. Use this illustration in lower-limb anatomy and vascular modules to contrast superficial versus deep venous pathways, or in phlebology and vascular surgery materials to support preprocedural planning of small saphenous reflux treatment and postoperative complication discussions.
