Description
Anterior neck anatomy is rendered with the common carotid artery ascending within the carotid sheath, medial to the internal jugular vein and deep to the platysma. Lateral to the vascular bundle, the sternocleidomastoid defines the surface contour while deeper infrahyoid and suprahyoid musculature frames the carotid triangle. Superiorly, the artery bifurcates near the level of the upper border of the thyroid cartilage into the internal carotid artery, which continues posterolaterally toward the skull base without cervical branches, and the external carotid artery, which remains more anteromedial and gives off named branches in the neck. Veins are depicted in blue, with the external jugular typically crossing superficial to sternocleidomastoid and the internal jugular coursing alongside the carotid arteries. Orientation to the carotid bifurcation matters in everyday procedural anatomy. This is the landmark region for carotid endarterectomy, where plaque at the carotid bulb can narrow the internal carotid origin and raise ipsilateral stroke risk; it is also where the carotid sinus baroreceptors and carotid body chemoreceptors sit, explaining reflex bradycardia with sinus massage or surgical traction. Needle and scalpel paths are unforgiving here. An anterior approach also helps teach why the hypoglossal nerve and ansa cervicalis are at risk during dissection in the carotid triangle, and why the vagus nerve remains posteriorly placed within the sheath between artery and vein.
