Description
Emerging from the external carotid artery in the upper neck, the facial artery courses anterosuperiorly toward the inferior border of the mandible, then hooks around the mandibular margin just anterior to the masseter before entering the face. Across the cheek it follows its characteristic tortuous path, running deep to the platysma and risorius region and then ascending toward the angle of the mouth and lateral nose. Superiorly, the vessel continues toward the medial canthus where it becomes the angular artery, lying lateral to the nasal cartilages and medial to the zygomatic region. Bony landmarks of the male skull and mandible frame the vessel’s relationship to the maxilla, teeth, and lower facial skeleton. That winding course is not decorative, it accommodates mandibular movement and facial expression while maintaining flow. Clinically, the facial artery is a key landmark for controlling hemorrhage from lower facial lacerations and for planning incisions in submandibular and perioral surgery. At the inferior mandibular border, its palpable pulse point just anterior to the masseter is a common teaching target, and its terminal angular segment is relevant in midface and nasal reconstruction where anastomoses with the ophthalmic system can permit retrograde embolic complications during filler injections. Use this anterior head and neck view in gross anatomy and head and neck surgery curricula to teach external carotid branching patterns, surface anatomy, and vessel-bone relationships that are hard to appreciate from angiography alone. It also fits well in ENT, maxillofacial, and plastic surgery publications discussing flap design (including facial artery musculomucosal flaps) and operative exposure in the submandibular triangle.
