Description
Arising from the external carotid artery in the carotid triangle, the facial artery courses superiorly and anteriorly in the neck, passing deep to the posterior belly of the digastric and stylohyoid before entering the submandibular region near the submandibular gland. It then hooks around the inferior border of the mandible just anterior to the masseter, a palpable crossing point, and runs obliquely across the cheek toward the lateral nose and medial canthus. Along its sinuous facial course, the vessel lies deep to the platysma and muscles of facial expression while remaining superficial to the buccinator and the body of the mandible, with accompanying facial veins shown in blue for contrast. Bony landmarks anchor the pathway. This trajectory matters because the facial artery is the dominant inflow to the superficial midface and a frequent source of brisk bleeding in lacerations at the mandibular margin and nasolabial fold. Its relationship to the mandible and masseter frames common hemostatic maneuvers and surgical planning for submandibular gland excision, facial flap design, and neck dissection where the external carotid branches must be identified and protected. In aesthetic practice, intravascular injection during filler placement near the angular artery can lead to skin necrosis and, via anastomoses with the ophthalmic circulation, vision loss.
