Description
Arising from the common iliac artery at the pelvic brim, the external iliac artery courses inferiorly and laterally along the medial border of psoas major toward the inguinal ligament, where it continues as the femoral artery. An anterior perspective typically places the external iliac vein medial to the artery, with the iliopsoas muscle mass posterior, and the inferior epigastric and deep circumflex iliac arteries branching anteriorly near the inguinal region. Bony context from the ilium and superior pubic ramus frames the vessel’s path, while the upper thigh musculature sits inferior and distal to the inguinal ligament. Surgical anatomy here matters because the external iliac artery is a primary inflow conduit to the lower limb and a frequent target for exposure in pelvic and vascular procedures, including iliac endarterectomy, bypass grafting, and proximal control for hemorrhage. Its relationship to the inguinal ligament and the femoral nerve (lateral to the iliac vessels on the iliopsoas) guides safe dissection, and its proximity to the external iliac vein explains why venous injury can complicate anterior approaches to the pelvis.
