Deep Circumflex Iliac Artery

Description

Running along the anterolateral abdominal wall, the deep circumflex iliac artery arises near the inguinal region and sweeps laterally toward the anterior superior iliac spine, then tracks posteriorly along the inner lip of the iliac crest. Rectus abdominis lies medial with its tendinous intersections and anterior rectus sheath, while the external oblique, internal oblique, and transversus abdominis occupy progressively deeper and more lateral planes. Inferiorly, the iliopsoas forms a muscular bed in the iliac fossa, and the iliac crest and adjacent pelvic girdle provide the bony contour that the vessel parallels. Venous counterparts in blue accompany the arterial course and drain toward the external iliac vein. Surgeons care about this artery because it defines a dependable pedicle for deep circumflex iliac artery (DCIA) flaps used in mandibular and maxillary reconstruction, and its perforators can support iliac crest osteocutaneous harvest.
