Description
Running anterolaterally across the proximal thigh, the lateral femoral circumflex artery branches from the profunda femoris (or proximal femoral artery) and gives rise to its descending branch, which tracks inferiorly along the lateral aspect of the femoral shaft. Distally, this descending artery courses between rectus femoris and vastus lateralis, staying lateral to the main femoral artery in the femoral triangle and continuing toward the lateral knee, where it participates in the genicular anastomosis. Bony landmarks frame the pathway, from the pelvis and proximal femur superiorly to the distal femur, tibial plateau, and leg bones inferiorly. Clear left to right orientation is maintained, with the tibia medial and the fibula lateral. Anterior mapping of this vessel matters because the descending branch is a primary feeder of the vastus lateralis and an established pedicle for the anterolateral thigh (ALT) flap, a workhorse option in head and neck reconstruction. Surgeons planning flap harvest or lateral thigh approaches need a dependable sense of where the vessel runs relative to the femur and quadriceps interval. Injury or variant anatomy can also affect collateral flow around the knee, which becomes relevant when the superficial femoral artery is compromised and genicular networks sustain distal perfusion.
