Description
Arising from the profunda femoris (deep femoral) artery in the proximal thigh, the lateral femoral circumflex artery courses laterally toward the region of the greater trochanter before giving off its descending branch. From an anterior perspective, the descending artery then runs inferiorly along the anterolateral femur, tracking between the quadriceps muscle compartment (classically near vastus lateralis and rectus femoris) and the lateral intermuscular septum as it approaches the knee. The femoral artery and vein occupy a more medial, anterior position in the femoral triangle, while the patella sits anterior to the distal femur at the knee joint. Bony landmarks anchor the vascular map. This vessel matters because the descending branch is the dominant pedicle for the anterolateral thigh (ALT) flap and a frequent target in perforator-based reconstruction of head and neck, breast, and lower-extremity defects. Anatomy varies. Surgeons often encounter origin variants from the femoral artery rather than the profunda, and the course relative to the vastus lateralis can affect perforator selection and dissection time. Clear depiction of the artery’s proximal takeoff near the greater trochanter and its distal run toward the lateral knee also supports planning around iatrogenic injury during lateral femoral approaches and minimally invasive hip or femur procedures.
