Running along the anterolateral thigh, the descending branch of the lateral femoral circumflex artery is traced from its origin near the profunda femoris region and directed inferiorly toward the lateral aspect of the knee. From a lateral perspective, it courses deep to the fascia lata and alongside the vastus lateralis, with the femoral shaft posterior to it and the patella positioned anteriorly at the distal end of the femur. Lateral landmarks such as the greater trochanter proximally and the iliotibial tract superficially provide orientation as the vessel descends. Arteries are distinguished from adjacent venous channels, with cutaneous and muscular nerve trajectories running in parallel planes. This artery matters when you need to understand perfusion of the anterolateral compartment and the vascular anatomy encountered during approaches to the proximal femur. Orthopedic exposure for intertrochanteric fracture fixation, total hip arthroplasty, or a direct lateral approach can place the descending branch and its perforators at risk where they traverse the vastus lateralis, a common source of persistent postoperative thigh hematoma when injured. It is also a key pedicle for the anterolateral thigh (ALT) flap, where preoperative mapping targets its septocutaneous or musculocutaneous perforators to the skin paddle. Bleeding here is brisk.
