Arising from the lateral circumflex femoral artery, the descending branch is traced along the posterolateral thigh, coursing inferiorly toward the lateral aspect of the knee. Its terminal arterial contributions spread around the distal femur and proximal tibia, with smaller arterioles feeding adjacent musculature and periosteum. Posteriorly, the vessel lies deep to the iliotibial tract and runs in the interval between vastus lateralis and the short head of biceps femoris as it approaches the lateral genicular region. Bony landmarks include the pelvis superiorly, the femoral shaft centrally, and the tibia and fibula distal to the knee joint, with accompanying veins returning proximally in parallel. Posterior orientation matters for flap planning and for understanding collateral pathways around the knee. The descending branch of the lateral femoral circumflex artery is a common pedicle for the anterolateral thigh flap and the vastus lateralis muscle flap, and its caliber and course help you predict perforator location even when you approach from the lateral or posterior side. It also participates in the genicular anastomosis, a practical concept when teaching why distal limb perfusion can persist after femoropopliteal disease or surgical ligation.
