Description

Arborizing across the lateral face, the facial nerve (cranial nerve VII) is presented after its emergence from the parotid region, dividing into its terminal branches that course superficially toward the forehead, periocular region, midface, and perioral musculature. Superiorly, temporal rami travel toward the frontalis and orbicularis oculi, while zygomatic and buccal branches sweep anteriorly over the zygomatic arch and cheek toward the upper lip and nasal ala. Inferiorly, the marginal mandibular branch tracks along the inferior border of the mandible toward depressor anguli oris and depressor labii inferioris, and a cervical branch descends into the upper neck toward platysma. Parotid gland topography is implied by the classic fan-like branching pattern. Clear landmarks. In practice, this branching anatomy is where facial nerve morbidity happens. Parotidectomy and facelift (rhytidectomy) dissections place the temporal and marginal mandibular branches at predictable risk zones, and even modest traction or cautery can translate into brow ptosis or lower lip asymmetry. Bell palsy teaching also benefits from a branch-level view, because incomplete recovery often leaves synkinesis along zygomatic and buccal distributions, and targeted botulinum toxin injections depend on knowing the likely course of these rami relative to the zygomatic arch and mandibular margin.
