Description

Arising from the pontomedullary junction, the facial nerve (cranial nerve VII) courses laterally with the nervus intermedius toward the internal acoustic meatus on the petrous part of the temporal bone, where it lies anterosuperior to the vestibulocochlear nerve (CN VIII). From this lateral perspective, the labyrinthine segment leads to the geniculate ganglion at the first genu, then the nerve turns posteriorly as the tympanic (horizontal) segment above the oval window region and continues inferiorly as the mastoid (vertical) segment toward the stylomastoid foramen. Distal branches typically fan within the parotid gland, dividing into temporofacial and cervicofacial divisions and then into the temporal, zygomatic, buccal, marginal mandibular, and cervical branches across the lateral face. Short and crowded anatomy. For otologic and skull base work, this trajectory through the fallopian (facial) canal is the map that matters. The tympanic and mastoid segments define danger zones in mastoidectomy and tympanoplasty, and their relationship to the lateral semicircular canal, incus, and stapes explains why iatrogenic facial palsy is a feared complication when disease or drilling breaches the canal. Bell palsy localizes proximal to the stylomastoid foramen when stapedius weakness and taste disturbance accompany facial weakness, and the geniculate ganglion is a classic site for varicella zoster reactivation in Ramsay Hunt syndrome. Use this lateral facial nerve plate for teaching temporal bone anatomy in gross anatomy, neuroanatomy, and ENT modules, and for illustrating operative corridors in otology texts, atlas figures, and patient education around parotid surgery or facial reanimation planning. It also suits radiology captioning when correlating CT of the temporal bone and MRI of the internal auditory canal with the expected course of CN VII.
