Description
Emerging from the foramen ovale in the greater wing of the sphenoid, the mandibular nerve (CN V3) descends into the infratemporal fossa as a thick mixed trunk, positioned inferior to the middle cranial fossa floor and anterior to the external acoustic meatus. Laterally, its course relates to the ramus of the mandible, while medially it neighbors the tensor veli palatini region and the cartilaginous auditory tube. Shortly after exit, small meningeal and muscular branches arise, and the main trunk divides into an anterior division (predominantly motor) and posterior division (predominantly sensory) that will distribute to the muscles of mastication and to the lower face, teeth, and tongue. Clear landmarks. Clinically, this lateral angle is the one you want when teaching why V3 can be injured at the skull base and how its branches become targets for dental and maxillofacial anesthesia. The inferior alveolar nerve, lingual nerve, and auriculotemporal nerve are all downstream of this segment, so pathology at or near the foramen ovale can present as combined jaw weakness (motor to masseter, temporalis, pterygoids) with sensory loss in the mandibular teeth, lower lip, and anterior two thirds of the tongue. Surgeons also use these relationships when planning approaches to trigeminal schwannomas or perineural spread from head and neck malignancy via the mandibular division. Use this illustration in head and neck anatomy teaching for dental, medical, and speech and hearing science curricula, and in operative atlases discussing infratemporal fossa, skull base, and trigeminal nerve procedures.
