Iliacus Beneath the Skin

Description

Positioned within the iliac fossa on the internal surface of the ilium, the iliacus fans out superiorly and laterally and then narrows inferiorly as its fibers converge toward the tendon of iliopsoas. Medial to the anterior superior iliac spine, the muscle belly lies anterior to the hip joint capsule and lateral to psoas major, with the combined tendon passing deep to the inguinal ligament to insert on the lesser trochanter of the femur. Adjacent bony landmarks include the iliac crest superiorly and the proximal femur inferiorly, and a partial lumbar spine may appear posterior and medial as a reference for pelvic orientation. Overuse or acute strain of the iliopsoas complex can produce anterior hip pain and snapping hip, and a prominent lesser trochanter insertion helps explain why apophyseal avulsion injuries occur in adolescent athletes and why metastatic lesions occasionally present as lesser trochanter pain in adults.
