Description
Running along the internal surface of the ilium, the iliacus occupies the iliac fossa with fleshy fibers fanning superiorly toward the iliac crest and converging inferomedially to join the tendon of psoas major as the iliopsoas. The muscle sits anterior to the sacroiliac region and lateral to the lumbar vertebral bodies, while the proximal femur and acetabulum lie inferior and slightly lateral at the hip joint. Posterolaterally, the gluteus medius and gluteus minimus cap the outer table of the ilium, framing the iliac wing that separates these abductors from the deeper iliacus. Bone landmarks are clear. The ilium, ischium, and sacrum define the pelvic ring. An iliacus-focused plate is most useful when you need to teach how hip flexion force is transmitted across the anterior hip, and how that differs from the stabilizing abductor function of gluteus medius and minimus on the lateral ilium during single-leg stance. Clinically, iliopsoas tendinopathy and internal snapping hip are often discussed at the level of the lesser trochanter, but the problem starts proximally, where the iliacus and psoas share a common tendon and glide over the anterior capsule and iliopectineal eminence. The relationship to the lumbar spine and iliac fossa also matters for femoral neuropathy from retroperitoneal hematoma or iliopsoas compartment pathology, since the femoral nerve typically courses between psoas major and iliacus before passing deep to the inguinal ligament. Use this artwork in gross anatomy teaching on the pelvis and hip, in orthopedic and sports medicine texts covering hip flexor mechanics, and in surgical education for anterior approaches to the acetabulum where the iliopsoas and iliacus are mobilized to protect neurovascular structures.
