Description
Centered within the thorax, the trachea descends in the midline and bifurcates at the carina into the right and left main bronchi, each entering the hilum on the medial surface of its lung. The right lung sits slightly more superior and broader, organized into superior, middle, and inferior lobes by the horizontal and oblique fissures, while the left lung is narrower with a single oblique fissure separating superior and inferior lobes and a medial cardiac impression. Inferiorly, both lungs taper into concave diaphragmatic bases that rest on the respiratory diaphragm, with their apices projecting superior to the clavicular level in anatomical position. Semi-transparency keeps the organs in situ over the chest wall. Orientation is immediate. Clinically, a superimposed anterior thoracic placement clarifies why aspirated material and endotracheal tubes tend to preferentially enter the right main bronchus, which is shorter, wider, and more vertical than the left. That same geometry informs bronchoscopy, one-lung ventilation, and interpretation of lobar collapse patterns, for example right middle lobe atelectasis from mucus plugging or compression near the hilum. The relationship of the left lung to the cardiac silhouette also helps explain the reduced left lung volume and the anterior positioning of the lingula.
