Description
Anterior thoracic anatomy is rendered with the female mammary gland shown in section over the pectoralis major, whose fibers form the muscular bed posterior to the retromammary space. Lobules and lactiferous ducts sit within adipose tissue and fibrous septa (suspensory ligaments), converging toward the nipple and areola on the anterior surface. Medial lymphatic vessels are traced coursing from the parasternal portion of the breast toward the sternal border, running in the superficial fascia and along perforating vessels that connect to the internal thoracic (parasternal) lymph nodes. Blue venous channels accompany the glandular parenchyma, giving a clear contrast between vascular and lymphatic pathways. Medial lymphatic drainage is where breast anatomy stops being academic and starts directing real decisions. Tumors in the medial quadrants frequently drain to parasternal nodes, which can be occult on physical exam and may not be sampled by a standard axillary sentinel node approach, altering staging and radiotherapy fields. The parasternal route also explains contralateral nodal spread across the midline and underpins why internal mammary node evaluation is discussed in medial breast cancers and in reconstructive planning when internal mammary vessels are exposed for free flap anastomosis. A common pathway, often missed. Educators can drop this plate straight into breast surgery teaching (sentinel node mapping, axillary versus internal mammary basins), oncology modules on lymphatic spread, or anatomy courses covering superficial fascia, pectoral region planes, and thoracic wall lymph nodes.
