Description
Medial lymphatic channels course from the lobules and lactiferous ducts of the female mammary gland toward the parasternal region, tracking along the medial quadrants of the breast and converging on lymph nodes adjacent to the internal thoracic (internal mammary) vessels. Superficial lymphatics within the dermis communicate with deeper vessels in the glandular parenchyma, while the retromammary space separates the posterior breast from the fascia overlying pectoralis major. Deep to the gland, pectoralis major and pectoralis minor provide a clear posterior boundary for the illustrated lymphatic pathways. Orientation matters. Drainage to the internal mammary (parasternal) nodes explains why tumors of the medial breast can metastasize across the midline and why contralateral nodal disease can appear even when the ipsilateral axilla is clinically negative. This medial route also underpins internal mammary sentinel lymph node mapping and influences radiotherapy field design, where coverage of the parasternal chain may be considered in selected patients with centrally or medially located carcinomas. The relationship to the pectoral fascia and intercostal spaces helps clarify potential routes toward anterior intercostal nodes and, less commonly, the pleura.
