Description
Arising from the internal carotid artery, the middle cerebral artery (MCA) courses laterally into the Sylvian fissure, then divides into superior and inferior trunks that fan over the lateral convexity of the hemisphere. Distal cortical branches run posteriorly and superiorly across the temporal and parietal lobes, tracking along the gyri and sulci toward the supramarginal and angular regions. Alongside the arterial tree, superficial cerebral veins drain in parallel trajectories, while selected cranial nerves and the brainstem appear inferiorly as anatomical orientation landmarks. Arteries are distinguished from veins by color coding for fast visual parsing. MCA territory is the day-to-day geography of stroke neurology. Occlusion at the M1 segment typically produces contralateral face and arm weakness and sensory loss, and dominant-hemisphere involvement often brings aphasia, while non-dominant lesions bias toward hemispatial neglect. Branch-level anatomy also matters for aneurysm patterns at the MCA bifurcation and for neurosurgical corridors that split the Sylvian fissure to access insular lesions or hematomas without sacrificing cortical branches. Small branches matter.
