Description
Arising from the internal carotid artery at the base of the brain, the middle cerebral artery (MCA) courses laterally into the lateral sulcus (Sylvian fissure) before dividing into superior and inferior trunks that fan out over the lateral cerebral hemisphere. Cortical branches spread superiorly across the frontal and parietal opercula and inferiorly over the superior temporal gyrus, tracking along the gyri and sulci that define the convexity. Proximal perforators (lenticulostriate arteries) project medially and superiorly from the M1 segment toward the basal ganglia and internal capsule. Big territory. This lateral trajectory makes the MCA the workhorse vessel in acute ischemic stroke, where embolic occlusion at the M1 segment or proximal M2 divisions produces the classic pattern of contralateral face and arm weakness, hemisensory loss, and gaze deviation, with aphasia on the dominant side or hemispatial neglect on the nondominant. The same anatomy matters in subarachnoid hemorrhage and aneurysm work, since MCA bifurcation aneurysms sit near the insula and Sylvian fissure, a corridor frequently opened in pterional and orbitozygomatic approaches. Perforator preservation is the margin, lenticulostriates do not forgive.
