Description
Cut in a lateral cross section through the orbit, the globe sits anteriorly within the bony cavity formed by the frontal bone superiorly, maxilla inferiorly, zygomatic bone laterally, and ethmoid bone medially, with the optic nerve coursing posteriorly toward the optic canal. Branches of the ophthalmic artery track forward from the orbital apex, giving off muscular arteries that run parallel to the extraocular muscles, typically accompanying the rectus muscles on their global and orbital surfaces. Small arterioles arborize across the periorbita and orbital fat, while venous channels, including tributaries of the superior and inferior ophthalmic veins, interlace around the nerve and muscle cones. Fine vascular detail dominates. Clinically, the muscular branches of the ophthalmic artery matter because they contribute to the anterior ciliary arteries, a major source for the anterior segment and a key consideration during strabismus surgery on the rectus muscles, where multiple muscle disinsertions can precipitate anterior segment ischemia. Proximity to the optic nerve sheath also frames how orbital hemorrhage or iatrogenic injury during retrobulbar anesthesia can threaten vision by raising intraorbital pressure and compromising perfusion. This is surgical anatomy. The accompanying ethmoid air cells and medial orbital wall are a reminder of the corridor used in endoscopic medial orbital decompression and the pathway for infection spread from sinusitis.
