Presented in a sectional cut through the orbit and anterior skull base, the globe sits anterior to the optic nerve as it courses posteriorly toward the optic canal, with the ophthalmic artery entering the orbit alongside the nerve after arising from the internal carotid artery. Branching arterioles spread anteriorly and circumferentially over the sclera and into the orbital fat, with companion venous channels in blue tracking a similar route toward the superior and inferior ophthalmic veins. Surrounding osseous landmarks include the bony orbit and adjacent paranasal sinus spaces, shown as hollow cavities medial and inferior to the orbital contents. Tight quarters. This corridor matters because the optic canal and annulus of Zinn create a fixed passage where vascular compromise and compressive neuropathy can coexist, a relationship that becomes clinically concrete in orbital apex syndrome, traumatic optic neuropathy, and optic nerve sheath meningioma. The ophthalmic artery’s proximal segment is also a key waypoint in endovascular planning: embolization of the external carotid system can threaten vision through anastomoses to the ophthalmic circulation, and inadvertent occlusion can produce acute, painless monocular vision loss through central retinal artery ischemia. Spatial understanding here prevents complications.
