Description
Cut through the cranial cavity, the dural venous sinuses stand out in blue against the inner table of the calvaria, with the superior sagittal sinus running in the midline along the attached margin of the falx cerebri toward the internal occipital protuberance. At the occiput, the occipital sinus occupies the falx cerebelli and descends in the posterior cranial fossa toward the foramen magnum, where it communicates with the confluence of sinuses and the marginal sinus around the rim of the foramen. Laterally, the paired transverse sinuses course along the attached margin of the tentorium cerebelli in the occipital grooves of the squamous occipital bone, continuing toward the sigmoid sinuses. Bone landmarks remain readable, including frontal, parietal, and occipital bones and the posterior cranial fossa contours. Posterior fossa venous anatomy is easy to mis-teach because the occipital sinus is variable, often small or duplicated, yet it becomes clinically relevant when dominant drainage patterns develop in infancy or in the setting of jugular bulb or transverse sinus hypoplasia. Neurosurgeons pay attention to this channel during midline suboccipital craniectomy and foramen magnum decompression, where dural opening near the internal occipital crest risks brisk bleeding from the occipital or marginal sinus. Small vessel, big consequence.
