Description

Superior to the brainstem, the paired optic nerves (cranial nerve II) course posteromedially toward the midline where their fibers decussate within the optic chiasm. From the chiasm, the optic tracts continue posterolaterally along the diencephalic region, sitting rostral to the origin of the oculomotor nerves on the ventral midbrain. Other cranial nerves emerge segmentally from the brainstem in symmetric pairs, with thicker mixed nerves arising from the pons and medulla and smaller bundles clustering near the pontomedullary junction. Clean midline anatomy. Attention to the chiasm matters because it is where lesion localization becomes clinically actionable: compression from a pituitary macroadenoma in the sella turcica classically produces bitemporal hemianopia by injuring crossing nasal retinal axons, while optic neuritis more often affects the prechiasmatic optic nerve and presents with unilateral painful vision loss. Aneurysms of the internal carotid artery or anterior communicating artery can distort the optic apparatus in a different pattern, and this superior brainstem orientation helps clarify why symptoms track fiber geography rather than eye position. Fiber sorting is the point.
