Description
Anterior orbital anatomy is centered on the globe, with the optic nerve (cranial nerve II) appearing as a thick, cylindrical bundle exiting the posterior pole of the eyeball at the optic disc and coursing posteriorly toward the optic canal. The nerve’s fascicular organization can be inferred from its smooth trunk, formed by tightly packed retinal ganglion cell axons rather than peripheral nerve fibers. Around the posterior globe, you would expect the proximal relations to include the sclera and the sleeve of dura and arachnoid that continue along the nerve, with the extraocular muscle cone lying more peripheral and lateral. Clinical work often hinges on this segment because it links ophthalmic findings to intracranial disease. Raised intracranial pressure transmits through the optic nerve subarachnoid space and can produce papilledema at the optic disc, a bedside clue that changes the urgency of neuroimaging. For surgeons and proceduralists, the posterior globe and optic nerve axis define the safe trajectory for retrobulbar and peribulbar anesthesia, where inadvertent optic nerve sheath penetration can lead to brainstem anesthesia or optic neuropathy.
