Description
Posterior thoracic wall anatomy is presented in a male figure with the vertebral column centered in the midline and the ribs curving laterally from their costovertebral joints. Inferior to the scapulae and deep to the superficial back muscles, the serratus posterior inferior is expected to span obliquely from the lower thoracic and upper lumbar spinous processes and thoracolumbar fascia to the posterior aspects of ribs 9 to 12, lying superior to the posterior layer of the thoracolumbar fascia and adjacent to the erector spinae. In the lower back, the quadratus lumborum is highlighted bilaterally, positioned lateral to the lumbar vertebral bodies and medial to the 12th rib and iliac crest. Serratus posterior inferior is routinely misunderstood as a primary respiratory muscle; this view helps clarify its deep placement and fascial continuity with the thoracolumbar fascia, which is more relevant to force transmission across the thoracolumbar junction than to rib elevation. Clinically, the interface between the 12th rib, quadratus lumborum, and thoracolumbar fascia is a common pain generator in posterior flank and low back syndromes, and it is a key landmark region for posterior approaches to the kidney and for ultrasound-guided quadratus lumborum blocks.
