Running obliquely across the lower thorax, the serratus posterior inferior occupies the posterolateral back at the thoracolumbar junction, lying deep to latissimus dorsi and inferior to the serratus posterior superior. From its vertebral origin on the spinous processes of T11 to L2 (via the thoracolumbar fascia), its muscle slips course laterally and slightly superiorly to attach near the inferior borders of ribs 9 to 12, lateral to the erector spinae mass. A posterior full-body view keeps the thoracic cage, vertebral column, and posterior pelvis in register, so the relationship of the muscle to the lower ribs and lumbar spine reads clearly. For teaching, this muscle is often misunderstood because it sits on the boundary between back musculature and the respiratory cage. It is a frequent source of confusion in differentiating thoracolumbar pain from costovertebral or costotransverse joint irritation, and it is also a practical landmark when discussing posterior approaches to the lower ribs and pleura, where rib 12 and the inferior rib angles orient needle or incision placement.
