Centered just inferior to the occipital bone, the rectus capitis posterior major is highlighted deep to the posterior cervical fascia and subcutaneous tissue, running superomedially from the spinous process of C2 (axis) to the lateral part of the inferior nuchal line. Laterally, its fibers border the obliquus capitis inferior as both muscles frame the suboccipital region, while more superficial layers include the splenius capitis and the upper fibers of trapezius crossing posteriorly toward the midline. Deep bony landmarks such as the posterior arch of C1 (atlas), the C2 spinous process, and the occipital squama anchor the muscle’s attachments and clarify its short, thick profile.
