Description
Beneath a translucent skin and hair layer in lateral profile, the rectus capitis posterior major is isolated within the deep posterior neck, running superomedially from the spinous process of C2 (axis) to the lateral part of the inferior nuchal line of the occipital bone. Posterior to the auricle and mastoid region, superficial cervical layers such as trapezius and splenius capitis give way to the suboccipital group, where rectus capitis posterior major sits medial to obliquus capitis superior and inferior and deep to semispinalis capitis. Bony context includes the occipital squama and the upper cervical vertebrae, orienting the muscle’s short, thick fiber direction relative to the atlanto-occipital and atlanto-axial joints. Deep anatomy is clear. Rectus capitis posterior major is a key driver of ipsilateral head rotation and extension at C1 to C2, and its position makes it a reliable landmark when teaching the suboccipital triangle and the course of the vertebral artery on the posterior arch of C1. In clinic, pain generators around this compartment often present as cervicogenic headache, and accurate surface-to-depth understanding helps when discussing C2 dorsal ramus involvement and why greater occipital neuralgia can coexist with suboccipital muscle hypertonicity.
