Description
Sectioned through the male kidney, the renal cortex forms a peripheral rim around the medulla, where multiple renal pyramids converge toward papillae and minor calyces before opening into the renal pelvis and proximal ureter. Along the medial hilum, the renal artery enters anterior to the renal pelvis, with the renal vein returning anteromedially, and segmental branches course toward the corticomedullary junction. Fine renal nerves from the renal plexus track with the periarterial connective tissue, their delicate fibers wrapping the renal artery and following its divisions toward the parenchyma. Small ganglia may be suggested near the arterial origin. Periarterial renal innervation is the practical anatomy behind renal sympathetic modulation of renin release, sodium handling, and renal blood flow, so seeing the nerves in direct relationship to the renal artery matters. Catheter-based renal denervation targets these fibers within the adventitia of the main renal artery and proximal branches, and this sectional perspective helps explain why accessory renal arteries and early branching can complicate effective ablation.
