Description
Running distally from the femoral triangle, the saphenous nerve branches from the femoral nerve and travels within the adductor canal, then becomes superficial between the sartorius and gracilis near the medial knee. Along the medial leg it courses with the great saphenous vein, giving infrapatellar and crural cutaneous branches that descend anterior to the medial malleolus toward the medial foot. At the ankle and hindfoot, the medial malleolus, talus, calcaneus, and medial metatarsals provide hard landmarks for the nerve’s subcutaneous terminal distribution, while accompanying vessels track in parallel. Orientation is medial and anteromedial. Superficial. For regional anesthesia, this nerve matters because it supplies sensation to the anteromedial knee, medial leg, and medial foot while sparing motor function, making a saphenous (adductor canal) block a preferred adjunct for knee arthroscopy, total knee arthroplasty, and medial malleolar or forefoot procedures. Its close relationship to the great saphenous vein explains why venipuncture or vein harvesting can produce dysesthesia or neuroma pain along the medial tibial border. The distal cutaneous field also helps differentiate saphenous sensory loss from L4 radiculopathy or tibial nerve injury when mapping postoperative numbness around the ankle. Use this asset in lower-limb anatomy teaching to trace the femoral nerve’s terminal sensory branch from the adductor canal to the medial ankle, and in anesthesiology or orthopedic publications illustrating landmark and ultrasound-guided approaches for adductor canal and distal saphenous blocks.
