Description
Running along the medial leg, the saphenous nerve is traced as the terminal cutaneous branch of the femoral nerve, coursing inferiorly with the great saphenous vein after emerging from the adductor canal. Distally, it passes anterior to the medial malleolus and continues onto the medial border of the foot, sending superficial branches over the tarsus toward the first metatarsal and hallux. Bony landmarks such as the tibia, medial malleolus, calcaneus, talus, navicular, cuneiforms, and metatarsals anchor the nerve’s position relative to the ankle and midfoot joints. Relationships are clear. Nerve and vessel run in close company. Clinically, this medial perspective matters because the saphenous nerve is a frequent casualty of procedures that also target the great saphenous vein, including vein harvesting for coronary artery bypass grafting and endovenous ablation for varicosities. Distal sensory symptoms often localize to the medial ankle and medial foot, and this course explains why patients can present with neuropathic pain or numbness after medial tibial plating, ankle arthroscopy portals placed too medially, or trauma around the medial malleolus. The same anatomy guides a saphenous nerve block at the level of the knee or ankle, where proximity to the vein becomes a practical landmark and a hazard.
