Description
Arcuate vessels course across the dorsum of the male forefoot, with dorsal metatarsal arteries running distally over the intermetatarsal spaces and dividing near the metatarsophalangeal joints into paired dorsal digital arteries for adjacent toes. Along the lateral (fibular) border, branches align with the fifth metatarsal and lateral tarsal region, while more medial channels track toward the hallux and first intermetatarsal space. Superficial dorsal veins (blue) accompany the arterial tree, forming dorsal digital veins that converge proximally toward the dorsal venous arch. Osseous landmarks, including the tarsals, metatarsals, and phalanges, frame the neurovascular pathways and the joint lines at the MTP and interphalangeal levels. Clear landmarks. Dorsal forefoot vascular anatomy matters when planning incisions and flap design, because the dorsal skin and extensor apparatus tolerate little ischemia and the dorsal metatarsal arteries can be the dominant inflow to the dorsal digital arteries when plantar contributions are limited. This perspective is also practical for correlating pulse assessment and Doppler mapping with the course of dorsalis pedis into the arcuate artery, and for anticipating vascular compromise in diabetes, thromboangiitis obliterans, or following metatarsal fractures and dislocations that can injure the intermetatarsal vascular bundles.
