Description
Arising from the dorsalis pedis artery on the dorsum of the foot, the dorsalis metatarsal arteries course distally across the metatarsus in the intermetatarsal spaces, running on the dorsal aspect of the metatarsals deep to the extensor tendons and toward the bases of the proximal phalanges. Each vessel gives dorsal digital branches to adjacent toes, with distal perforating branches passing plantarly to contribute to the plantar arterial arch. Superficial dorsal veins accompany the arterial network, while slender cutaneous nerves travel in parallel, consistent with the distribution of the superficial fibular (peroneal) nerve and the deep fibular nerve in the first web space. Bony landmarks, from metatarsal shafts to phalangeal segments, provide a fixed scaffold beneath the soft tissues. A dorsal perspective is the working view for many clinical encounters because the dorsalis pedis pulse, palpated lateral to the tendon of extensor hallucis longus, serves as a rapid assessment of distal perfusion in peripheral arterial disease, diabetes, and after trauma. The branching pattern of the dorsalis metatarsal arteries also explains why dorsal incisions over the intermetatarsal spaces, common in approaches for Lisfranc injuries or metatarsal fixation, demand careful planning to avoid iatrogenic vascular compromise and delayed wound healing. Small vessel detail matters here. End-artery territories in the toes do not forgive careless dissection.
